Brookside Cardinal Soccer
Before being eligible to participate in practices, scrimmages or games, the following forms must be filled out and turned in to a member of the coaching staff. In addition, fees must be paid before playing time is allowed.
OSHAA Physical Form
New physical forms are required each year. If you had a physical just prior to the spring sport season, it may still be on file. Otherwise, plan on getting a new physical. Physicals for band or for work permits are acceptable.
Coach's Expectation Form
Read, understand and sign. The purpose of this form is to help you understand what this program is about, how it will run and what you need to do to be personally successful.
Sheffield-Sheffield Lake Annual Information Form
Fill out and return. This form is for school records to make sure they have the latest information about you on file. FOUR Copies are needed.
Fill out and return. This form gives parental consent to participate in the sport of soccer, with an understanding that certain risks are involved and an injury potential exists.
Socks and Transportation Fees
Fifty dollar ($50) Bus Fee. This fee covers the cost of the mandatory school bus transportation to and from away regular season and tournament soccer contests as well as preseason scrimmages. Checks are to be made out to "Sheffield-Sheffield Lake City Schools".
Thirteen dollar ($13) Sock Fee. This fee covers the cost of two pairs of socks to be worn on game day. By purchasing socks through the soccer program, the uniform consistency remains intact. These socks are yours to keep. The rest of the uniform is the property of the Brookside soccer program and must be turned in at the conclusion of the season. Checks are to be made out to "Cardinal Athletic Boosters".
